Annual Reviews

Patients who have a long term condition will be called at least annually to be reviewed. 

We recommend all patients with a long term condition purchase a blood pressure monitor to monitor their blood pressure at home. The British Heart Foundation has very useful information on how to take a blood pressure reading. Click here to visit their website. 

Click here to see the British Hypertension Society list of recommended blood pressure monitors. There are many options around £30, and this can save you trips to the Practice to have a reading taken.

If you are unable to obtain a blood pressure monitor, please do not worry! We will take a reading in clinic at your annual review appointment, however we prefer a week's worth of readings where possible in order to calculate your average blood pressure over a period of time. 

To submit your home blood pressure readings, please record them for a week prior to your appointment, and send them to us before your appointment via eConsult here

Diabetes Reviews

If you have Diabetes (plus any other long term condition) you will need to book a Purple Clinic appointment with one of our Diabetic Nurses.

Please ensure you complete a weeks worth of home blood pressure readings in advance of this appointment. You can submit your readings here. If you are unable to do so, the nurse will check your blood pressure.

Following this clinic appointment, the Diabetic Nurse will contact you 1-2 weeks later to discuss your results, and answer any questions you may have.

Mental Health Review

If you have a mental health condition (plus any other long term condition) you will need to book a Blue Clinic appointment with one of our Practice Nurses.

Please ensure you complete a weeks worth of home blood pressure readings in advance of this appointment. You can submit your readings here. If you are unable to do so, the nurse will check your blood pressure.

Following this clinic appointment, your usual GP will contact you 1-2 weeks later to discuss your results, and answer any questions you may have.

Asthma or COPD Review

If you have Asthma or COPD you will be contacted by Maria, Respiratory Nurse Specialist. She is currently undertaking reviews over the phone, or by video. 

Spirometry is currently suspended due to COVID.

If you have another long term condition, please look at the relevant information on this page, as you may need a review by another member of the team.

Other Annual Reviews

If you have once of these conditions (and do not have one of the conditions listed above)

  • Hypertension
  • Chronic Kidney Disease (CKD or Renal Disease)
  • Rheumatoid Arthritis
  • Atrial Fibrillation
  • Coronary Heart Disease (CHD)
  • Heart Failure
  • Peripheral Arterial Disease (PAD)
  • Stroke or TIA

you will need to book a Yellow Clinic appointment with one of our Healthcare Assistants.

Following this clinic appointment, the Practice Nurse will contact you 1-2 weeks later to discuss your results, and answer any questions you may have.

Dementia and Learning Disability Reviews

You will be invited for these reviews by our Admin Lead for Dementia or Learning Disabilities. They will contact you directly to arrange this.

If you have other long term conditions listed above, the Admin Lead will help to coordinate this with your usual GP when they contact you.

Contraceptive Pill Reviews

If you are due a contraceptive pill review, please submit an eConsult here. You may need to attend for a blood pressure reading, or you can use our blood pressure monitors in Reception.
