Sickness certificates

If you are unwell and unable to work because of illness for a period of seven working days or more, your doctor can issue you with a note for your employer. As of April 2010, this is now known as a "fit note"rather than a "sick note". Please note that your doctor needs to either have seen you personally, or must be able to refer to written documentation from another doctor who has seen you in order to provide this note for you.

For periods of abstinence less than seven working days, you are able to self-certify time off work. Either contact your employer directly, or complete the self-certificate form SC2 which is available at reception, from your employer, or you can download a copy here. If your employer insists on a fit note within the first seven days of your absence, this is a non-NHS service and so your doctor will issue a private sickness certificate, for which the fee is currently £20, payable by your employer.
